Lisa Von Lehmden

CEO

Lisa Von Lehmden embarked on her journey in Northeast Ohio in 2004, joining Infinity Home Health Services as an education liaison. She played a pivotal role in expanding the organization’s reach and impact by successfully managing the marketing and development of her colleagues’ care delivery models of over 600 patients under both Ohio Passport/Waiver and Medicare Skilled, showcasing her leadership abilities. Her passion for home care and hospice services led her to a role as a board member of the Ohio Council for Homecare and Hospice. Lisa’s commitment was further recognized when she was appointed as the Board Chair. 

Currently serving as the Executive Director of the Ohio Council for Home Care and Hospice, Lisa continues to shape the future of home-based healthcare. And a never changing environment and landscape, Lisa recognized in an ever changing environment and landscape, the need for focusing on culture for clinicians and experience for patients, as a result, she now holds the position of CEO at Luxe Healthcare @ Home, based in Westlake, Ohio. In this role, Lisa leverages her extensive experience and expertise to ensure the delivery of high-quality, compassionate care to individuals in the comfort of their homes. 

Amy Ruffner

Administrator

Amy Ruffner, RN, WCC, is a seasoned healthcare professional with over 30 years of experience, specializing in home healthcare. Her career began at Cleveland Clinic Home Healthcare, following two years of service at the main campus in the ENT step-down unit. Amy then transitioned to EMH Home Healthcare, where she spent 16 years in various capacities, including field clinician, Clinical Liaison, Intake specialist, and Preceptor.

Seeking a new challenge, Amy joined Ideal Home Health Care as both a field clinician and Wound Care Navigator, where she earned her Wound Care Certification (WCC) and excelled in managing wound care programs. When Ideal closed, she took on the role of Director of Home Healthcare at the Visiting Nurse Association (VNA), where her leadership skills continued to shine.

Today, Amy is the Administrator and Clinical Director at Luxe Healthcare @ Home, having played an integral role in its launch. A devoted servant leader, Amy is passionate about delivering exceptional patient care while empowering her team of clinicians to ensure both patient satisfaction and staff fulfillment.

Jody Peterson

Director of Operations

Jody Peterson brings over a decade of experience and dedication to the home healthcare industry. Beginning her journey in 2013 at Ideal Home Health, she gained invaluable expertise in intake, insurance authorizations, payroll, and other administrative functions. Transitioning into the field as a Marketing Liaison, she discovered her passion for business development and built strong connections that advanced her career.

As Director of Business Development for both Ideal Home Health and, later, the Visiting Nurse Association of Ohio, Jody played a pivotal role in driving organizational growth and enhancing patient care. In 2022, she embraced an exciting new challenge: assisting in the launch of Luxe Healthcare @ Home.

Now serving as Director of Operations, Jody is committed to fostering a positive agency culture that empowers employees, delivers exceptional patient care, and supports the surrounding community. Her leadership reflects her unwavering dedication to innovation, compassion, and excellence in home healthcare.

Amanda Likavec

Director of Rehabilitation

Amanda Likavec, PT, is a seasoned physical therapist with 23 years of experience. She began her career traveling and working as a contract therapist in various settings, including in-patient, out-patient, and skilled nursing facilities (SNF). Twenty years ago, Amanda transitioned into home healthcare, joining Just In Time Therapy as a home health therapist. It was during her time there that she earned her COS-C certification and took on a management role as Director of Rehab.

Amanda’s passion for patient care led her to join Ideal Home Health Care, an agency she had collaborated with, where she continued to provide exceptional physical therapy services. Following Ideal’s merger with the Visiting Nurse Association (VNA), Amanda remained with the organization, gaining valuable insights into what makes an agency strong—consistent dedication from every clinician, manager, and administrator.

Now, as the Director of Rehab at Luxe Healthcare @ Home, Amanda is committed to building a team where every therapist cares deeply about their patients and colleagues alike. She strives to create a supportive and collaborative environment, ensuring that Luxe’s rehabilitation department delivers the highest quality of care.

Crystal Deeds

Clinical Liaison

Crystal Deeds, LPN, is a dedicated and compassionate nurse with over 17 years of professional nursing experience and 28 years in healthcare. She began her journey as a caregiver at Harbor Court Assisted Living, where she spent a decade supporting residents and taking on various roles, including receptionist and housekeeping. After completing nursing school, Crystal accepted a position at Harbor Court, advancing from charge nurse to Assistant Director of Nursing (ADON), a role she held for many years.

Crystal’s career continued to evolve as she became a Clinical Liaison at Ideal Home Health Care. Following a merger with the Visiting Nurse Association (VNA), she continued her liaison role, later moving on to serve as a Clinical Manager at The Ganzhorn Suites Memory Care. Her passion for patient care, particularly in home health, led her to her current position at Luxe Healthcare @ Home, where she ensures patients receive seamless, compassionate care during their transition from healthcare facilities to the comfort of their own homes.

Morgan Hammond

Business Development

Morgan Hammond, LNHA, is a seasoned healthcare professional with over 15 years of experience in the industry. A graduate of Bowling Green State University with a Bachelor’s degree in Healthcare Administration, Morgan began her career in long-term care, working in the business office. Her passion for helping residents and their families inspired her to further her education, and she earned her Administrator’s license to enhance her knowledge and leadership in the field.

Throughout her career, Morgan has held diverse roles in long-term care, including business office management, admissions, case management, and administration. Her well-rounded expertise has allowed her to make a meaningful impact on both the operational and patient care aspects of healthcare.

In 2024, Morgan joined the Luxe Health Care at Home team as a Business Development Liaison. In this role, she leverages her extensive experience to help families navigate the complexities of the healthcare system, ensuring that their loved ones receive the highest quality care in the comfort of their own homes.
